Codex app crashes with "ignoring interface.defaultPrompt: maximum of 3 prompts is supported"

Keep getting handshake error if I try to reload or start codex again. Only fix is to restart the machine (even trying to stop wsl is not working properly, wsl -l -v hangs till I restart the machine).

What steps can reproduce the bug?

Ran it overnight. This happened on long-running agent work, it crashed this way for two nights in a row. Feedback code is attached, could not find any other pattern.
Edit: Three nights in a row now, this crash happens in the morning.
