Review feedback doesn't render the findings

What issue are you seeing?

The agent was writing the findings from the review, but at the end of each finding wrote, it just gone. It was 3-4 findings and it don't render, just show the last message from the reviewer.

What steps can reproduce the bug?

Ask the agent to proceed with a review in codex review mode.

What is the expected behavior?

Should show all the findings, typically through the UI Cards.
