"Failed to set goal" as a red pop-up, every time.
What steps can reproduce the bug?
In an existing chat, change to pursue goal, then try it.
What is the expected behavior?
It should try to pursue a goal or provide a better experience on what did not work

I’m seeing the same issue as well. The goal-setting feature does not work correctly in my Codex app either: attempts to set or pursue a goal fail instead of applying successfully.
This has been happening for me since the day the goal feature first launched, and it still has not been fixed in later app updates. It would be helpful if the team could take another look, since this seems to be a persistent issue rather than a one-off problem.
go to your config.toml, find the [ features ] (for me its at line 99) and add "goals = true" somewhere there
you can find the config.toml on your pc or through settings in the configuration tab
This feature is now enabled by default in the latest version, so you should no longer see this problem.
