Now the Codex app keeps launching calc.exe??

What issue are you seeing?

Codex app with approval policy set to "On request", sandbox = "elevated" (the default), and no "trusted" projects in the config.toml

What steps can reproduce the bug?

Download or clone https://github.com/pointed-stick/surprise and open it up in the codex app. Send any message in the chat window, and calculator will pop up??

What is the expected behavior?

I was expecting to inspect the contents of the repo safely, but the repo got added to my trusted projects automatically. How can i vibe like this??
