Windows desktop regression: long "thinking" hangs, reconnect behavior, failed child process snapshot, and turn-state desync after update

I have another strong reproduction case that seems to confirm this is not just slow model latency or a pure network issue.

In this task, Codex Desktop had already performed part of the file edit and the UI showed a concrete code diff in progress:

• admin-server.js +29 -15

However, the task never properly finished and remained stuck on "thinking" for about 4 hours 33 minutes.

So in this case:

1. local tool/file editing had already happened
2. the UI showed concrete edit progress
3. but task state did not advance to completion
4. the app remained stuck indefinitely until manual intervention

This makes the issue look even more like a task/turn-state progression bug after local tool execution, rather than normal model slowness.

It is consistent with the other log signatures I reported earlier:

• Item not found in turn state
• Failed to collect child process snapshot
• slow mcpServerStatus/list

What steps can reproduce the bug?

1. 在 Windows 上打开 Codex Desktop。
2. 进入一个项目目录，执行一个较长或较复杂的任务。
3. 该任务不一定必须是重启项目，也可能是代码修改、分析、修复、运行验证等多步骤任务。
4. 在部分任务中，Codex 会先实际执行一部分本地操作或文件编辑。
5. 随后界面长时间停在“thinking / 正在思考”，有时还会出现 reconnecting。
6. 手动点击 Stop 再 Continue，很多时候任务又能继续推进。

补充说明：

• 该问题不是只在启动/重启项目任务里出现，其他长任务、工具较多的任务、甚至部分纯文本任务也可能出现。
• 我已经完整重装 Codex 后再次复现，问题仍然存在。

What is the expected behavior?

预期行为是：任务应在本地工具调用、代码编辑或分析完成后正常推进到结束状态，而不是长时间停在“thinking”。

如果任务已经完成了部分编辑或执行了本地命令，界面应该正常收尾并返回结果，而不是一直挂起，直到用户手动 Stop / Continue 才恢复。

• Windows Desktop
• Codex version: 26.519.11010.0
• clean reinstall performed: yes
• issue still reproduces after reinstall: yes

已尝试但无效：

• 重装工作区依赖
• 软重置本地状态/缓存
• 卸载并重装 Codex
• 卸载 360画报 / 360pingbao
• 卸载 Microsoft PC Manager

手动测试结果：

• Get-CimInstance Win32_Process 返回很快（约 0.177 秒）
• Get-CimInstance Win32_PerfFormattedData_PerfProc_Process 返回很快（约 0.403 秒）

但日志中仍反复出现：

• Failed to collect child process snapshot
• mcpServerStatus/list 耗时异常偏高
• Item not found in turn state
