Codex Desktop on macOS: AppShot capture succeeds but attaching fails with “unable to attach appshot”

What version of the Codex App are you using?

Codex 26.519.81530 (3178)

What platform is your computer?

macOS 15.7.7, Apple Silicon

What issue are you seeing?

The Codex Desktop App fails to attach an AppShot with the error:

unable to attach appshot

Screen capture permission appears to be granted, and the AppShot capture flow appears to run, but the captured AppShot is not attached to the conversation.

What did you try?

  • Re-enabled Screen Recording / Screen & System Audio Recording permissions for:
  • Codex
  • Codex Computer Use
  • Reset ScreenCapture TCC permissions for:
  • com.openai.codex
  • com.openai.sky.CUAService
  • Quit and reopened Codex multiple times
  • Cleared Codex frontend/image/cache directories
  • Reinstalled Codex
  • Confirmed the installed Codex version is the latest in the appcast
  • Confirmed macOS screen capture itself can work outside the failing AppShot attach flow

Relevant diagnostic notes

System logs showed TCC Allow for screen capture and AppShot-related capture windows appearing/closing, so the failure seems to happen after capture, during the attach/insert step.

Expected behavior

AppShot should be captured and attached to the current conversation.

Actual behavior

Codex reports:

unable to attach appshot

and no AppShot is attached.
