Codex.app QR linking from ChatGPT iOS stalls for account formerly tied to Team workspace

What happened?

ChatGPT iOS -> Codex.app QR-code linking does not complete for an account that was previously associated with a Team/Teams workspace and later continued as an individual/personal account using the same email address.

The QR code is scanned on the iPhone, but the flow does not proceed afterward. Repeated retries and app/Mac restarts do not resolve the issue.

A separate, straightforward account created directly and signed in with Google SSO can complete the same iPhone-to-Mac Codex.app linking flow successfully, so this appears account-state dependent rather than a general local network, QR scanner, or device issue.

Account history that seems relevant

• The affected email address was originally used primarily with a Team/Teams workspace account.
• Later, an individual/personal account was added/used with the same email address.
• The Team/Teams account/workspace was then discontinued/removed.
• The affected account still signs in with Google SSO.
• After that history, ChatGPT iOS -> Codex.app QR linking never progresses after QR scan.
• A clean/simple Google SSO account without that workspace history links successfully on the same general setup.

No email address, workspace ID, or account-identifying details are included here because this is a public issue. If OpenAI support needs account-specific identifiers, I can provide them through the private Help Center support channel.

Expected behavior

After scanning the Codex.app QR code in ChatGPT iOS, the mobile-to-Mac linking flow should continue and connect ChatGPT on iPhone to Codex.app on macOS.

Actual behavior

The QR code is accepted/scanned on iPhone, but the linking flow does not continue. The Mac remains unconnected.

Steps to reproduce

1. Use an OpenAI account that was formerly associated with a Team/Teams workspace and later continued as an individual/personal account using the same email address.
2. Sign in to Codex.app on macOS.
3. Start the mobile-to-Mac linking flow in Codex.app and display the QR code.
4. Open ChatGPT on iPhone using the same affected account.
5. Scan the QR code from the iPhone.
6. Observe that the flow stalls and does not complete.

Notes

This looks like a stale account/workspace/device-linking/auth state issue specific to accounts that previously had Team/Teams workspace association under the same email address.
