Codex forces phone re-verification multiple times per active session

What issue are you seeing?

The authentication flow frequently drops active sessions and requires a refresh. It can require multiple refreshes within a single hour, and every single time it refreshes, it forces a new phone verification.

The session should remain active after verification without interrupting work

Additional information

This happens on the exact same device and within the same active browser/CLI session, without using a VPN or changing network connections. The system sometimes even requires phone verification multiple times in less than an hour
