What issue are you seeing?

When selecting text in a Codex App response, the contextual toolbar with actions like Add to chat and Ask in side chat sometimes does not appear.

This seems more likely when the selection spans multiple lines and includes structured content, code/text blocks, or UI mockup-style text. The text is highlighted, but the action toolbar is missing, so there is no way to add the selected text back into the chat from the contextual action.

What steps can reproduce the bug?

Open a Codex App chat response with multiline text.
Select text across several lines.
Include part of a structured text/code block or UI mockup-style block in the selection.
Look for the contextual toolbar with Add to chat / Ask in side chat.

What is the expected behavior?

The contextual selection toolbar should appear consistently whenever selectable response text is highlighted, including multiline selections and selections that cross into text/code blocks.

Additional information

A related context-selection issue exists at #20950, but that issue is about repository/file context selection. This issue is specifically about selected response text not showing the contextual Add to chat toolbar.

This appears to be the same issue already described in #25263.

I can reproduce it consistently on Windows: the “Add to chat / Add to conversation” bubble works only at 100% zoom, but disappears at any non-100% zoom level.

I am seeing a similar selection-action regression on macOS.

Environment:

Codex Desktop: 26.527.31326 (CFBundleVersion 3390)
Platform: macOS 26.3.1 build 25D2128, arm64

Observed behavior:

Selecting text in Codex Desktop no longer reliably exposes the contextual action used to add the selection into the conversation. The selected text is highlighted, but the Add to chat / Add to conversation affordance may not appear, so the selection cannot be inserted back into the chat as the usual selected-text block.

Expected behavior:

When selectable text is highlighted, Codex should consistently show the selection action affordance so the selected text can be added to the current conversation.

I am adding this as another data point rather than opening a duplicate. I have not narrowed this to a root cause yet, but from the user-facing symptom it looks related to this issue family.

Still reproducible on a newer macOS Codex Desktop build.

Environment:

Codex Desktop: 26.602.40724 / CFBundleVersion 3593
Codex CLI: 0.137.0-alpha.4
macOS: 26.5 build 25F71
Hardware: Apple Silicon

Observed behavior:

Selecting text in assistant output is unreliable for conversation actions. Sometimes the text is visually highlighted, but the contextual Add to chat / Ask in side chat action does not appear, so I cannot continue a conversation from selected output text as before.

This is not limited to the older builds mentioned above; it still reproduces on 26.602.40724.

Related local state at the time of observation:

threads: 484 total, 432 active, 52 archived
active title/preview/first_user_message metadata: ~2,082,708 chars
~/.codex/sessions: ~1.2 GB
~/.codex/state_5.sqlite: ~3.7 MB
~/.codex/logs_2.sqlite: ~831 MB

I am also seeing Desktop performance pressure with multiple Codex windows/threads open, but the selected-text action regression is user-visible independently.
