“Add to chat” selection toolbar does not appear for some selected response text

What issue are you seeing?

When selecting text in a Codex App response, the contextual toolbar with actions like Add to chat and Ask in side chat sometimes does not appear.

This seems more likely when the selection spans multiple lines and includes structured content, code/text blocks, or UI mockup-style text. The text is highlighted, but the action toolbar is missing, so there is no way to add the selected text back into the chat from the contextual action.

What steps can reproduce the bug?

1. Open a Codex App chat response with multiline text.
2. Select text across several lines.
3. Include part of a structured text/code block or UI mockup-style block in the selection.
4. Look for the contextual toolbar with Add to chat / Ask in side chat.

What is the expected behavior?

The contextual selection toolbar should appear consistently whenever selectable response text is highlighted, including multiline selections and selections that cross into text/code blocks.
