Compaction regularly throwing away recent work, uses prior work as starting point

What version of the Codex App are you using (From “About Codex” dialog)?

Version 26.527.60818 (3437)

What subscription do you have?

Pro

What platform is your computer?

Darwin 24.6.0 arm64 arm

What issue are you seeing?

I am frequently running into this scenario:

  • User: do X
  • Codex: (does X)
  • User: do Y
  • Codex: ok I will do Y
  • (compaction)
  • Codex: ok I will do X

Probably since a few days ago, this has happened nearly every time it's compacted when starting new work (I can't recall a time it did not do this)

Example I just hit:

<img width="535" height="867" alt="Image" src="https://github.com/user-attachments/assets/12a4352e-ae44-459f-ab7a-6f589b9d8370" />

What steps can reproduce the bug?

Complete a task, then ask to complete a new task just before compaction.

What is the expected behavior?

It does not lose track of the current work and revert to prior work it already did
