What steps can reproduce the bug?
Running a custom Action that launches a PowerShell script causes:
- "AttachConsole failed"
- "Unable to find Electron app"
- "Cannot find module ... type=action&action=1"
The same PowerShell script runs successfully when executed directly outside Codex.
What is the expected behavior?
