Codex in ChatGPT iOS app goes stale and stops streaming

Bug report

Codex in the ChatGPT iOS app appears to go stale and stop streaming progress updates without any visible indication that the connection has disconnected or fallen behind.

Environment

• App: ChatGPT iOS app
• Device: iPad Mini
• Feature: Codex session thread / streaming progress view

Expected behavior

When a Codex session thread is open in the ChatGPT iOS app, the stream should keep updating as Codex makes progress, or the UI should clearly indicate that the stream has disconnected/stalled and provide an automatic or manual reconnect path.

Actual behavior

The thread stops updating silently. There is no obvious stale/disconnected indicator. To get updates streaming again, the user has to swipe up to leave the app and reopen it, after which the session reconnects and starts streaming updates again.

Reproduction notes

1. Open a Codex session thread in the ChatGPT iOS app on an iPad Mini.
2. Keep the app open as a portable always-on view of Codex progress.
3. Wait while Codex is expected to keep streaming progress.
4. Observe that the stream eventually stops updating without indication.
5. Swipe up to go home, then reopen the app.
6. Observe that the stream reconnects and resumes updating.

Impact

This makes the iOS Codex view unreliable as an always-on progress monitor. Users can mistakenly believe Codex has stopped making progress when only the client stream has gone stale.

Suggested fix direction

Please make the iOS Codex thread stream reconnect reliably when it becomes stale, and/or add a visible stale/disconnected state with a reconnect action.
