Codex Desktop Windows: Ctrl+Shift+V plain-text paste inserts clipboard content twice

I am on Version 26.623.61825 • Released 29 Jun 2026 on Windows 11, still not fixed!!!!!!!!

Still reproducible on OpenAI.Codex 26.814.5167.0 x64 on Windows (tested 2026-08-20).

The conflicting Start or stop voice mode binding for Ctrl+Shift+V was removed in Settings → Keyboard Shortcuts, and the app was fully restarted. After that, Ctrl+Shift+V performs a plain-text paste, but inserts the clipboard contents twice.

Expected: the clipboard contents are inserted exactly once, without formatting.

This confirms the duplicate-paste bug remains in the current Windows build even after the voice shortcut conflict is explicitly removed.

Still reproducible on Codex Desktop for Windows, version 26.818.5229.0 x64.

Environment

• Windows 11 IoT Enterprise LTSC, build 26100, 64-bit
• Codex Desktop 26.818.5229.0
• Toggle voice chat shortcut explicitly unbound
• Codex fully quit and reopened after changing the shortcut
• The app updated during troubleshooting; the bug persists in the newer build

Reproduction steps

1. Open any chat in Codex Desktop for Windows.
2. Open Settings → Keyboard Shortcuts.
3. Remove the binding for Toggle voice chat if Ctrl+Shift+V is assigned.
4. Fully quit Codex, including its tray process.
5. Reopen Codex.
6. Copy a distinctive value such as paste-test-7f3a.
7. Focus the message composer and press Ctrl+V.
8. Observe that the value is inserted once.
9. Clear the composer.
10. Press Ctrl+Shift+V.
11. Observe that the value is inserted twice: paste-test-7f3apaste-test-7f3a.

Expected

Ctrl+Shift+V should perform one plain-text paste after the voice shortcut is unbound.

Actual

The clipboard contents are inserted twice. Regular Ctrl+V inserts them once.

Persistence

The issue remains after fully restarting Codex and after updating from version 26.818.4152.0 to 26.818.5229.0.

Diagnostic observation

The installed application bundle appears to contain both a native Electron Edit-menu paste handler and a composer paste handler that inserts clipboard content. It is possible that Ctrl+Shift+V reaches both paths on Windows, although this is an inference from the packaged code rather than confirmed instrumentation.

Reproduction

1. In Settings → Keyboard Shortcuts, unbind Ctrl+Shift+V from Toggle voice chat.
2. Fully restart Codex Desktop.
3. Copy Codex to the clipboard.
4. Focus the Codex message composer and press Ctrl+Shift+V once.
5. The composer inserts CodexCodex.
6. Pressing Ctrl+V inserts Codex only once.

I originally encountered this through Voxtype, which is configured to paste transcribed text using one synthetic Ctrl+Shift+V chord. Its logs show a single transcription and a single paste action, but Codex inserts the phrase twice. The manual clipboard reproduction above confirms that Voxtype is not required to trigger the bug.

Expected: Ctrl+Shift+V inserts the clipboard contents once after the voice-chat shortcut is unbound.

Actual: the clipboard contents are inserted twice.
