Documentation is incorrect

What is the issue?

Documentation says:

How do I update Codex?
Run codex --upgrade at any time to fetch the latest release.

This feature is not in fact implemented. Must use npm install -g @openai/codex

Version: codex-cli 0.23.0 (after upgrading from 0.22 with npm install command)
Operating System: Mac OS Sequoia 15.6

Either change the docs to remove reference to the --upgrade flag or implement it

Result: codex starts a session by examining how to upgrade my project:

I need to understand the user's message, "--upgrade." However, I don't have context for what they want to upgrade. It could refer to upgrading a package
with pip or dependencies in general. Since we're in the "ai-analyzer" directory under pangur-extensions, asking a clarifying question seems essential. I
think I'll explore the repo to figure out which package manager to use. First, I should run a command like ls -la to list the files. Let's start there!
