What issue are you seeing?

After a recent Codex update, Codex Desktop on my Windows machine can no longer launch any local commands. Even basic commands such as Get-Location, git status --short, and rg --files fail before execution with:
windows sandbox: runner error: CreateProcessAsUserW failed: 5
Codex also shows that permissions are locked by requirements.toml, so I cannot change the approval/sandbox setting from the UI.

What steps can reproduce the bug?
