Codex Code Review cannot be disabled and drains credits

What issue are you seeing?

We have Codex Code Review disabled and the toggle to not use credits off, but it still drains our codex subscriptions and credits.

What steps can reproduce the bug?

1. Enable Codex Code Review
2. Disable Codex Code Review
3. Toggle off Enable credits use

What is the expected behavior?

Credits should not be drained when Codex Code Review is disabled.

Nice to have: We can configure which model code review is using.
