Seeing auth_revoked errors when trying to use Linear Plugin in Codex Despite being connected to the plugin

What issue are you seeing?

auth_revoked message in session output for Linear app plugin despite the linear plugin being connected to codex.

What steps can reproduce the bug?

1 - Open the codex
2 - Ensure Linear Plugin is installed and connected
3 - Open a new chat session
4 - Add Linear Plugin to the chat session
5 - Use following prompt: Investigate and implement flaky zen tests listed in linear issues and convert it to Playwright or Jest: Add Linear Tickets ID or URL

What is the expected behavior?

retrieve linear ticket information and continue with analysis/implementation
