Codex entered the 7th Hell (never ending loop without completing the task AND consuming credits)

What issue are you seeing?

Codex stayed in a weird loop where it kept repeating itself, consuming credits and not completing the task. I eventually had to stop it.

I noticed this occurred after I had run of out credits and Codex had to stop in the middle of building when it hit limits. Then when I had the reset and asked it to resume it struggled and entered into this never ending loop mentioned above.

What steps can reproduce the bug?

Covered above. Make Codex hit limits in the middle of a build. Reset credits. Resume the task and watch it loop eternally without completing the process, consuming credits until exhaustion.

What is the expected behavior?

Codex resumes tasks after hitting limits without going into loops, completes the task(s) AND is efficient with credit usage.

Additional information

Please fix the issues with credit usage. I am seeing massive degradation across the board (slower, inefficient, hungry for tokens, doesn't complete the task, doesn't resume properly, etc.)
