App crashes immediately on launch on Windows 11 (26100.8655) — no WSL distro installed

What version of the Codex App are you using?
26.608.1337.0 (installed from Microsoft Store)

What subscription do you have?
Plus

What platform is your computer?
Microsoft Windows 11 x64, Build 26100.8655

What issue are you seeing?
The Codex app crashes immediately on launch. The window briefly appears and then closes with no error dialog.

Steps to reproduce:

  1. Launch Codex from the Start menu or taskbar
  2. App briefly appears in the taskbar
  3. App closes immediately with no error message

What I have tried:

  • Cleared app cache under %LOCALAPPDATA%\OpenAI\Codex
  • Full uninstall and reinstall from Microsoft Store
  • Disabled WSL feature via DISM
  • Added registry key HKCU\Software\Chromium with usagestats=0
  • Rolled back Windows Update KB5094126
  • Manually set runCodexInWindowsSubsystemForLinux=false in config file

Additional context:

  • WSL is not installed (wsl -l -v returns "no installed distributions")
  • Running codex.exe app-server directly works (process appears in Task Manager)
  • Electron log shows: Failed opening key Software\Chromium to set usagestats; result: 5
  • No Codex-related errors appear in Windows Event Viewer
  • The app was working fine until June 11, 2026
