Goal in Chat Causing Endless Token Burn with Hung Agent in Thinking

What issue are you seeing?

I have a conversation in Codex that was pursuing a goal, the agent is stuck in thinking, cannot be stopped in the UI stop button and it's burning tokens by thinking with no output and zero advancement. The agent does not respond to steering, and is actively running constantly.

What steps can reproduce the bug?

What is the expected behavior?

It does not get hung, advances on tool calls and response, and does not actively consume my token and usage without results.
