What issue are you seeing?

After the latest update, it's asking for approval for every single thing even when full access is provided. I run multiple sessions at a time and can't keep approving everything. Fix this asap.

We encounter the same issue since latest update. In "Approve for me" mode, Codex asks for vastly more permissions than before. Workstreams that ran smoothly before suddenly create many permission prompts.
(same sys specs, Version 26.616.31447)

Same issue here, solved by using this

# ~/.codex/config.toml
...

sandbox_mode = "danger-full-access"
approval_policy = "never"

and restarting vscode
