Locked Use plugin update failed on macOS Tahoe 26.5.1

What subscription do you have?

ChatGPT Pro

What platform is your computer?

Darwin 26.5.1 arm64 Apple Silicon (M1 Max)

What issue are you seeing?

When enabling "Locked Use" under Computer Use, Codex shows:

  • Unable to update Locked Use settings
  • Plugin update failed

Environment:

  • Codex App 26.616.41845
  • macOS Tahoe 26.5.1
  • MacBook Pro 16-inch (2021)
  • Apple M1 Max
  • 64 GB RAM
  • ChatGPT Pro subscription

What works:

  • Computer Use is enabled
  • Chrome integration works normally
  • Codex launches and functions normally

What does not work:

  • Locked Use cannot be enabled
  • The toggle immediately fails with "Plugin update failed"

Troubleshooting already performed:

  • Reinstalled Codex App
  • Reset Accessibility permissions:

tccutil reset Accessibility

  • Reset Screen Recording permissions:

tccutil reset ScreenCapture

  • Restarted Codex
  • Restarted macOS

Additional observations:
No Codex helper appears to be installed under:

  • /Library/PrivilegedHelperTools
  • /Library/LaunchDaemons

Expected behavior:

Actual behavior:
The installation/update fails immediately with:
"Unable to update Locked Use settings"
"Plugin update failed"

What steps can reproduce the bug?

When enabling Locked Use, Codex should successfully install or update any required system components and enable the Locked Use feature without errors.

What is the expected behavior?

  1. Open Codex App 26.616.41845 on macOS Tahoe 26.5.1.
  1. Open Settings.
  1. Navigate to Computer Use.
  1. Enable Chrome Integration (works correctly).
  1. Attempt to enable "Locked Use".
  1. Codex immediately shows:
  • Unable to update Locked Use settings
  • Plugin update failed
  1. The Locked Use toggle remains disabled.
