The Usage Remaining menu item should be driven by an explicit capability state, not transient render timing. Track whether the account-usage endpoint is available, last fetch status, plan/account scope, feature flag state, and unavailable reason.
If the item is hidden, the settings UI can still expose a disabled row with "usage unavailable: reason" and a timestamp. That makes it debuggable when the menu appears briefly then disappears after an async entitlement check.
