What subscription do you have?
Azure OpenAI API
What platform is your computer?
Darwin 25.3.0 arm64 arm
What issue are you seeing?
This happens randomly amidst a conversation with Codex, the issue is replicable on both Codex app and Codex in VS Code.
What steps can reproduce the bug?
This happens almost everytime in the middle of a conversation, not sure how can I help with exact reproduction steps.
What is the expected behavior?
The conversation should keep on going, right now I've to start a new chat and hope I can get significant work done before it interrupts the chat again.
