Visual Studio Code Plugin keeps showing error "Codex process errored: Codex process is not available"

What issue are you seeing?

In the macOS VS Code Codex Plugin, if a task is interrupted due to inputting task prompts, one of the following errors will occur, prompting a restart of the Plugin. Simultaneously, the Codex App functions normally:

  1. Codex process errored: Codex app-server process exited unexpectedly (code=unknown, signal=SIGTRAP). Last CLI error: rmcp::transport::worker: worker quit with fatal: Transport channel closed, when Auth(AuthorizationRequired)

  1. Codex process errored: Codex process is not available

rmcp::transport::auth: Token refresh not possible, re-authorization required. error=OAuth token refresh failed: Server returned error response: invalid_grant: Grant not found

What steps can reproduce the bug?

In the VSCode Plugin:

  1. Create a new session
  2. Enter task prompts
  3. GPT Start Thinking
  4. Error pop-up

What is the expected behavior?

Normal task execution
