Environment/context:

• Remote access: UU remote connection into a Windows machine
• Terminal: WezTerm on the remote Windows machine
• Scenario: copying plain text, not an image
• Symptom: pressing Ctrl+V in the Codex TUI reports Failed to paste image: no image on clipboard...

A useful detail is that this depends on the terminal/paste path:

• In traditional cmd.exe, mouse right-click paste worked for plain text.
• After switching to WezTerm, right-click did not paste by default.
• Pressing Ctrl+V in WezTerm appears to reach Codex as a raw key event, so Codex treats it as the image-paste shortcut instead of a text paste.
• Using terminal-owned paste bindings such as Ctrl+Shift+V or Shift+Insert is the workaround.
• Configuring WezTerm to bind Ctrl+V to PasteFrom 'Clipboard' also works because the terminal consumes the shortcut and sends text paste content instead of forwarding raw Ctrl+V to Codex.
