Summary

Codex suddenly started failing to display images/thumbnails correctly in the macOS app. Image attachments that should show visual previews now appear as gray placeholder tiles with the generic image icon instead of rendering the actual image.

What happened

While reviewing a Codex task that produced/inspected a report PDF, the UI showed multiple attachment/image preview tiles, but most of them did not load. They remained as dark gray placeholders with a generic image icon. One PDF thumbnail/preview appeared, but the surrounding image previews failed to render.

The issue appeared suddenly in the Codex app and affects the ability to visually inspect evidence/images inside the task output. In this case, the task specifically required manual visual confirmation of report pages, TOC/table pages, and image-based evidence; the broken image previews made that verification unreliable.

Steps to reproduce

Open a Codex app task or conversation that contains multiple generated/attached images or PDF/page thumbnails.
Wait for the task output to finish loading.
Observe the image preview grid.
Several image previews remain gray placeholder tiles with the generic image icon instead of loading the actual images.
Submit in-app feedback; the app returns Feedback ID 019efa69-9b74-7ab0-864b-a17f3921281a.

Expected behavior

All image attachments/thumbnails should render normally so the user can visually inspect the generated evidence and report pages.

Actual behavior

Most image preview tiles fail to render and only show generic placeholder icons. This makes image-based verification difficult or impossible from inside Codex.

Impact

This breaks workflows that rely on visual inspection, especially PDF/report verification tasks where the user must confirm page layout, blank pages, appendix entries, table boundaries, and attached evidence screenshots.
