Steps to reproduce

  1. Open Codex Desktop.
  2. Open VS Code with the Codex extension.
  3. Create a new thread from the VS Code extension.
  4. Observe that the new thread never appears in Codex Desktop.

Expected behavior

A thread created from the VS Code extension should appear in Codex Desktop automatically, just like a thread created from Desktop appears in VS Code.

Actual behavior

Synchronization is one-way:

  • ✅ Desktop → VS Code works.
  • ✅ If a thread is created in Desktop, VS Code can join it and all subsequent messages sync correctly.
  • ❌ VS Code → Desktop does not work for newly created threads.
  • ❌ Restarting Codex Desktop does not make the thread appear.

Additional observations

Once the thread originates from Desktop, synchronization is fully bidirectional.

The problem only affects discovery of newly created threads from the VS Code extension.
