Codex Windows sandbox repeatedly fails to launch command runner.

Observed sequence:

1. codex-windows-sandbox-setup.exe initially not found by bare filename.
2. Helper exists under .codex packages and can execute.
3. codex-command-runner.exe/resource lookup was inconsistent across:

• AppData\Local\Programs\OpenAI\Codex\bin
• .codex\packages\standalone\releases\0.142.2...
• WindowsApps OpenAI.Codex_26.616.10790.0

1. CodexSandboxOffline user was missing and had to be manually recreated.
2. After users existed and ACLs were granted, sandbox still fails:

CreateProcessAsUserW failed: 5
