Assistant messages are duplicated as progressively longer snapshots. For example, the CLI shows and/or persists output like:

I also see repeated full progress messages many times in a row.
This is not just a cosmetic display issue. The duplicated content appears to be persisted into the thread context. One affected thread became unusable with:

Sending a follow-up such as "continue" fails the same way:

Expected behavior:
A single assistant turn should be stored once. Streaming updates should be coalesced into one final assistant message, not persisted as multiple partial or cumulative assistant
messages.
Impact:
Long-running threads become unrecoverable because the duplicated transcript inflates the prompt beyond the model context limit. Starting a new thread works temporarily, but the
issue can reproduce again.
This seems closely related to the cumulative streaming/history persistence issues reported in #29755 and the CJK/context blow-up behavior reported in #26305.

One useful diagnostic split here is whether the provider is emitting several
response.output_item.doneevents for assistant messages where eachitem.content[0].textis a cumulative snapshot rather than a distinct completed item.The current client path appears sensitive to that shape: the SSE parser forwards each
response.output_item.doneas a completedResponseItem, and core records completed response items into history immediately. So if a Bedrock/Mantle stream sends:those can become three persisted assistant messages instead of one coalesced final message.
If this is reproducible, the most useful sanitized evidence would be the raw event sequence around one duplicated assistant turn: event
type,sequence_number,item.idor output index if present, itemstatus, role, phase, and text lengths only. Please avoid posting full prompt text, credentials, env vars, or unredacted logs. If the repeated snapshots share the same item identity, the likely client-side fix is to replace/coalesce prior snapshots for that item until the stream reaches the final completed item; if they have different IDs, this may need a provider-specific normalization or retry/dedup guard.
