Minimal reproduction

  1. Create a new empty project.
  2. Ask Codex:

Create hello.txt containing:

hello

This succeeds.

  1. Ask:

Read hello.txt and tell me its contents.

Expected

Codex reads the file and replies:

hello

Actual

Codex responds:

"I'll read the file directly from the workspace and report back."

It then remains in Thinking indefinitely.

No error is shown.

This reproduces consistently.

  • File creation works.
  • Workspace reads hang.
  • Brand-new project.
  • Internal NTFS SSD.
  • Reinstalled.
  • Reset.
  • Cleared cache.
  • WMI verified.
