Bug: Codex Window Title and Taskbar Label Do Not Reflect User-Set Conversation/Session Name on Windows

What issue are you seeing?

On Windows, the Codex application window title bar and the corresponding Windows taskbar label always display a fixed/generic name (e.g., "Codex" or "ChatGPT") and do not reflect the user-defined conversation/session title​ that was set inside the app.
This causes the following problems:
When multiple Codex windows are open, all show identical generic titles — making it impossible to distinguish which window belongs to which project or chat session.
The Windows taskbar tooltip and button label also show the generic app name instead of the active session title.
Even when conversations are renamed within Codex, the native OS window caption and taskbar entry remain unchanged.
Additionally, all Codex windows are currently bundled under a single taskbar icon, preventing individual reordering or ungrouped display — but the most urgent issue is that window/taskbar titles must reflect the user-set session name.

What steps can reproduce the bug?

Open the Codex desktop app on Windows.
Create or open a conversation and rename it (e.g., "Backend API – Refactor Auth").
Observe the Windows window title bar and taskbar label — they still show the default app name ("Codex" / "ChatGPT"), not​ the renamed session title.
Repeat with a second window using a differently named conversation — both show identical generic titles.

What is the expected behavior?

The native Windows window caption and taskbar button/tooltip should immediately update to match the current conversation/session title set by the user.
Each Codex window should display its own session name (e.g., "Codex – Backend API – Refactor Auth") so multiple open sessions can be visually distinguished on the taskbar and Alt+Tab switcher.
