Codex Crash

Codex desktop app crashes immediately after updating to version 26.623.42026 on macOS.

The crash occurs during plugin startup with:

(code=null, signal=SIGKILL)

The app repeatedly reports invalid bundled plugin manifests, including errors such as:

• interface.defaultPrompt: maximum of 3 prompts is supported
• prompt must be at most 128 characters

Affected plugin paths have included:

• template-creator
• ngs-analysis

I have already tried:

• rebooting macOS
• clearing Codex runtime caches
• clearing temporary plugin caches
• moving aside ~/.codex/config.toml
• disabling template-creator in config.toml
• rebuilding the Codex plugin/runtime cache

The issue persists even after the configuration/cache reset. My local Xcode project is unaffected.

Please investigate whether build 26.623.42026 has a bundled-plugin or plugin-loader regression on macOS.
