The crash occurs during plugin startup with:

Affected plugin paths have included:

template-creator
ngs-analysis

I have already tried:

rebooting macOS
clearing Codex runtime caches
clearing temporary plugin caches
moving aside ~/.codex/config.toml
disabling template-creator in config.toml
rebuilding the Codex plugin/runtime cache

The issue persists even after the configuration/cache reset. My local Xcode project is unaffected.

Please investigate whether build 26.623.42026 has a bundled-plugin or plugin-loader regression on macOS.

Codex Desktop continues to run.

Saved projects/pins validation passes.
Chrome/browser bundled manifests are present.

So on Windows Desktop this appears to be a bundled template-creator manifest/schema mismatch that currently logs a warning and ignores the 4th prompt, rather than a crash. It may be related to the crash reports here if other platforms treat the manifest warning as fatal.
