What issue are you seeing?

Summary
Codex crashes consistently while processing prompts that require code analysis or modification. The crash occurs in both the Codex CLI and the Codex App.
Environment
Codex CLI version: 0.142.3
Installation: Homebrew (/usr/local/bin/codex)
macOS: 26.5.1 (25F80)
Hardware: Intel MacBookPro16,2 (x86_64)
Authentication: ChatGPT account
codex doctor: All checks passed (17 OK, 0 warnings, 0 failures)
Reproduction
Example:
cd ~/Github/<git-directory>
codex

Prompt:
Would you add more detailed Javadoc comments to MyResportSpec?

Codex starts processing:
I’ll inspect TokenExchangeDelegationSpec and nearby spec classes...

Then immediately exits:
zsh: trace trap codex

The same occurs using:
codex exec "Add more detailed Javadoc comments to TokenExchangeDelegationSpec."

The command begins execution and then terminates with:
zsh: trace trap codex exec

The Codex App exhibits the same crash.
Expected Behavior
Codex should inspect the repository, update only the requested Javadocs, and return normally.
Actual Behavior
The process terminates with SIGTRAP before completing the task.
Diagnostics Performed
codex doctor reports a healthy installation.
Authentication succeeds.
WebSocket connection succeeds.
Repository detection succeeds.
Crash occurs after the model begins executing.
SQLite logs contain normal WebSocket traffic with no application-level errors immediately before the crash.
macOS Crash Report

EXC_BREAKPOINT (SIGTRAP)

Termination:
Trace/BPT trap: 5

Crash occurs while initializing the embedded V8 runtime.
Relevant stack frames include:
v8::base::OS::SetPermissions()

v8::Isolate::New()
codex_code_mode::runtime::spawn_runtime()

The crash report (.ips) is attached.
Additional Notes
The issue began immediately after upgrading/installing Codex 0.142.3.
The problem reproduces consistently across multiple repositories.
Both the CLI and the Codex App are affected.
Reinstalling Codex did not resolve the issue.
