What issue are you seeing?
Some time last week (probably around 06/23 or 24), the quality of code analysis and output dropped.
Even in a relatively small change, Codex would keep making mistakes and then finding new P1's and P2's during reviews. This process would repeat again and again.
What steps can reproduce the bug?

What is the expected behavior?
Before last week, Codex was working absolutely fine.
