Codex acting weird. Does not follow instructions. Takes very long. Not normal.

I got this:

Also, worth noting when it needs perm to run a script. For example I asked it to remove all python versions from my mac and reinstall a fresh copy from hombrew. It would ask for permission to do that (inside the vs code terminal and the regular terminal) and I wouldn't see an option to approve - I instinctively press enter and it works but the option isn't showing. Also, what if it needed my sudo pass to do admin work on the mac. There doesn't seem to be an option to enter a pass.
