What steps can reproduce the bug?
- Use Codex in "Ask for approval" mode.
- Wait for Codex to elevate a permission request.
What is the expected behavior?
I can see the command prefix that Codex offers to whitelist.
Additional information
I've seen this in the app on two different macOS machines, and (I think) in the VS Code extension, as well.
This has happened consistently across multiple recent Codex app versions for the past several days.
