Codex Web says it updated repository, but "Update Branch" doesn't appear

What issue are you seeing?

Sometimes Codex Web edits files and "I've updated the repository." However, the "Update Branch" button in the upper-right corner never appears. No commit reaches GitHub

Once a session enters this state, it never seems to recover.

What steps can reproduce the bug?

Windows 11, Firefox, Free plan of ChatGPT. Link to Github, link to a private organization, authorize to only one repo.

Start a new conversation. Ask Codex Web to edit files. Click the 'Create PR' and a pull request occurs.

Ask it to edit again, click the 'Update Branch' a commit of this PR occurs.

After Several edits, the 'Update Branch' does not show again.

What is the expected behavior?
