In the last week Codex intelligence dropped by a lot!

I have the details along with the session logs in the feedback (Feedback ID: 019f19da-678a-7c92-bd3d-53eec5434ada), but the gist is that Codex is spiraling into an infinite cycle of "fixes" that produce new P1's, P2's, and this morning P0's.

I've never observed this before and it all started around 2026-06-24.

What steps can reproduce the bug?
