Codex Windows App silently exits without user action.

Evidence:
Monitor observed 9 Codex-related processes at 20:28:59, then NO_CODEX_PROCESS at 20:29:14.
Windows AppModel later logged container destroyed at 20:29:51.
No Application Error, no System error, no AppXDeployment update event, no WER/Crashpad/Sentry error was found.

It is valid JSON and contains 0 NUL bytes, so my case does not appear to be caused by corrupted chat_processes.json.
