What issue are you seeing?

During a Codex session, Codex was working on my project files. Near the end of the task, Codex unexpectedly closed or disconnected. When I reopened it, I found that the project chat/thread had disappeared, and the files Codex created or modified were also gone from the project workspace.
This is very serious because I was working on a real project and had not yet pushed the code to GitHub. I understand that I should use Git backups, but Codex should not silently delete project files and the related project chat/thread.
