Questions asked by CodeX selected as "No answer provided" after very short timeout. I haven't even read through the questions and the descriptions of the provided options yet.

What steps can reproduce the bug?
I added "Ask me questions if you are unclear or needs decisions." in the prompt and used plan mode.
What is the expected behavior?
CodeX awaits for me to answer the questions.

I have the exact same issue. I didn't even get to read through the first question's choices it asked before it just said "No answer provided" for 3 questions and continued on...
Hope OpenAI get this fixed soon, or we have to rerun the request causing using more usage. Also, do you also experience the CodeX extension in VS Code stucked on the OpenAI LOGO, on the chat UI, review code differences and preview plan?
I have that issue on the ChatGPT Windows application instead of the IDE extension. I haven't tried the extension yet.

If this is a feature, it's super bad and shouldn't be forced on without any way to disable this. Codex doesn't often recommend the best solutions and there is no way to tell what it decided until it writes the plan and then you have revise things. It's a waste of time and tokens. Please remove this or make it Opt-In feature only.
@tolewhite looks like a feature, very bad feature because I see timer near questions (probably 2 minutes).
