Good. That rules out the hover card too.
What is confirmed:
Popout shortcut is unassigned.
Alt + Space does nothing.
The normal Quick Chat home card renders correctly.
No shortcut event was logged.
At the incident time, the app automatically created two quickChat renderer windows and logged repeated layout errors.
After the blank panel disappeared, it no longer existed in the visible Win32 window inventory.
The defensible diagnosis is a transient blank Chromium/Electron surface created during automatic Quick Chat prewarming or reloading. It was not caused by anything you pressed. Clicking it likely activated the Quick Chat renderer, explaining why integrated Chat opened.
Codex FIX:
Root cause: two orphaned Playwright headless Chrome daemons leaked blank compositor windows onto the desktop. They were exactly 780×580 at (10,10), matching your screenshot.
Stopped only the orphaned Playwright processes.
Their Chrome children exited.
Normal Chrome remains running.
No Playwright daemons remain.
The separate chat_processes.json corruption is also fixed.
