ChatGPT Desktop: Every new Codex task fails with "no rollout found for thread id"

What issue are you seeing?

Every new Codex task fails immediately with:

Error creating task
no rollout found for thread id ...

This happens for every newly created task.

I have already tried:

• Signing out and signing back in
• Restarting ChatGPT Desktop
• Creating multiple new threads
• Verifying that Codex CLI works correctly on the same machine and account

The issue only affects the ChatGPT Desktop Codex UI. Codex CLI works normally.

Environment:

What steps can reproduce the bug?

1. Open ChatGPT Desktop on macOS.
2. Switch to Codex.
3. Create a new Codex task.
4. Enter any prompt (for example: "Reply with OK only.").
5. Submit the task.

Result:
The task immediately fails with:

Error creating task
no rollout found for thread id ...

What is the expected behavior?

Every newly created Codex task immediately fails with:
"Error creating task"
"no rollout found for thread id ..."
