What issue are you seeing?

After sustained Codex use involving heavy thread management tool use, the Codex app will fail to create new threads with no concrete error but "loading..." See attached screenshot.

Creating new tasks on the same workspace using the codex CLI continues to work.

Killing the "Codex (Renderer)" process and letting it reload fixes the issue.

What steps can reproduce the bug?

Have been unable to pin down the exact conditions.
