New task button does nothing

What issue are you seeing?

In an /extension/panel/new tab, the upper right "New task" button no longer does anything at all. To create a new task, I need to close or archive the current task

What steps can reproduce the bug?

1. Cmd+Shift+P > "Codex: New Codex Agent"
2. Do anything with the agent, e.g. type "Who are you" and hit enter
3. Now try to create a new task by clicking the upper right "New task" button
4. Observe that nothing happens

What is the expected behavior?

1. The current task is added to history
2. The tab updates to a fresh new task
