What issue are you seeing?

The Codex desktop application crashes repeatedly while working on coding tasks.

Observed facts:

  • Windows Event Viewer reports codex.exe as the faulting application.
  • Exception code: 0xC0000409.
  • Windows automatically generated multiple crash dump (.dmp) files under:

%LOCALAPPDATA%\CrashDumps

Before opening this issue I investigated several possible causes:

  • config.toml appears normal.
  • Local API configuration appears normal.
  • Git repositories work normally.
  • Node.js and npm execute normally.
  • Codex sandbox initializes successfully.

Based on these observations, I could not identify a project configuration issue. The available evidence shows that codex.exe is terminating with exception 0xC0000409, but I cannot determine the root cause without internal symbols.

I have multiple Windows crash dump (.dmp) files available if they would help the investigation.
