I believe this happened to me yesterday July 17 as well. Worse, I can't use "/usage" to reset limits. Would like to use one of my remaining 3 resets but can't figure out a way to actually access them on any codex surface.
(Linux, Ubuntu, kitty terminal, latest codex CLI 0.144.5)
