Approve for me now denies everything

What issue are you seeing?
During GPT 5.5 times, I have never encountered an issue with the automatic approval.
Recently, when using the Codex app with GPT 5.6 Sol, the Approve for me has been very annoying with its denials, to the point of the feature being useless.
Example of a Auto-review denied high risk, when I use a skill to review code changes with claude -p:
This would send private worktree code to an untrusted external Claude service and refresh external auth, with no user approval for that third-party disclosure in the transcript.
I have seen the same kind of denial for git push from a worktree, which apparently needs elevation to touch git state outside of the worktree.
The denial reason was talking about not having verified the privacy of my GitLab repository, which has not changed, and where Codex has pushed successfully hundreds of times before.
I tried documenting in the skill that I have explicitly allowed the review to send the code to Claude. It still did not go through.
With these absurd denials, I am left with the options to either run Full access or switch to manual approval.
What steps can reproduce the bug?
Please test the sensitivity of the automatic approval review.
