What issue are you seeing?

When in /plan mode, questions steal the window focus, which can trigger unwanted answer submissions if the wrong key is pressed.

This is annoying for 2 reasons: it interrupts the user's current activity very forcibly, and unwanted answers might not match the user's desires, wasting time and usage in the process.

What steps can reproduce the bug?

  1. Switch or start any session to /plan mode
  2. Prompt Codex in a way that triggers questions
  3. Switch to a terminal or file window and type
  4. Question is asked -> focus is lost

What is the expected behavior?

  1. Switch or start any session to /plan mode
  2. Prompt Codex in a way that triggers questions
  3. Switch to a terminal or file window and type
  4. Question is asked -> notification of input required without focus loss
